Welcome and thank-you for agreeing to help us understand how to deliver the best physiotherapy care we can for you and your family. The survey starts with some information about you and moves onto questions about healthcare and physiotherapy.

It should take about 10 minutes on average and as a thank-you we’d like to enter you into our prize draw to win an Apple Watch which can be collected at Broadway Pharmacy with Cure Clinics on Friday 8th April 2022.
